Patient Intake Terms and Conditions
Version 1.4 | October 20, 2026
In short
Visentia runs this online check-in for your clinic. You confirm your appointment and update your details, and your clinic's care team reviews what you send before your visit.
If you don't agree, don't continue. Your clinic can help you check in another way.
1. Who provides this check-in
Visentia LLC ("Visentia") provides this online check-in on your clinic's behalf. Your clinic sent you a private link for one upcoming appointment so you can review and update your information before your visit.
2. How you agree
The first screen of the check-in links these Terms and our AI Policy. By selecting Continue, you agree to these Terms and to the AI Policy, including card reading when it is offered. Voice check-in is optional: speech processing starts only when you choose Start talking and allow microphone access. Opening a link or reading a document does not count as agreeing. Your current choice is remembered for this appointment; a new appointment or a changed version needs a new choice.
3. Confirming it's you
Before showing your information, we ask for your first and last name and your date of birth, and check them against the appointment your link was sent for. We then send a one-time code to the email address your clinic has on file or, if you choose, by text message to the mobile number your clinic has on file. Only enter a code that was sent to you.
4. What you can do here
- Confirm or tell us it isn't your appointment.
- Review and update your phone, email and address.
- Tell us the reason for your visit.
- Review your eye drops, other medications and allergies.
- Tell us about any symptoms.
- Confirm your insurance by scanning your card and typing its details, typing the details without a scan, telling us you have no active insurance, or asking clinic staff to help.
Some information is shown from your clinic's records so you can check it. Your answers are saved as you go. After you submit, you can't edit them here; contact your clinic if something needs to change.
When offered, an AI assistant can help fill in these same answers by voice. You can switch to the form without starting over. Review your answers before selecting Submit yourself. The assistant cannot sign clinic forms for you: electronic signing has its own statement and explicit signing action over the forms you select.
5. Using the check-in
- Use it only for the appointment the link was sent for.
- Enter information that is true as far as you know.
- Keep your link and one-time code to yourself.
- Don't try to see anyone else's information or interfere with how the check-in works.
6. What happens to what you send
Your clinic's staff review your answers and may correct them before using them. Your original answers remain part of the check-in record. Submitting your check-in does not itself change your medical record. After reviewing your answers, clinic staff decide what to update. They may enter approved changes themselves, or apply supported changes to your medical record electronically through Visentia.
When you submit, Visentia may also open a preliminary visit for your appointment in your clinic's records so staff can review your check-in. That visit does not mean you are checked in; your clinic completes your check-in.
7. Your privacy
Visentia handles your information on behalf of your clinic to verify your identity and support your check-in. Your clinic's Notice of Privacy Practices explains how your clinic uses and protects your health information.
Using this check-in or agreeing to these Terms does not allow anyone to train AI models on information that identifies you. Our AI Policy explains how Visentia may use de-identified information in the future.
8. Not for emergencies
This check-in is not monitored in real time. If you have a medical emergency, call 911. For urgent eye problems, call your clinic.
9. What this check-in does not do
- It doesn't give medical advice, diagnose or treat anything, and it doesn't replace talking with your care team.
- It doesn't confirm that your insurance covers your visit. Your clinic checks coverage separately.
- Information shown from your clinic's records may be out of date. If something looks wrong, tell your clinic.
- The check-in may sometimes be unavailable. If it is, your clinic can help you check in another way.
10. Our AI Policy
Our AI Policy explains optional voice check-in, card reading when offered, the information used by these services, and how to stop them. Neither service provides medical advice or replaces your care team.
11. If you don't agree to these Terms
If you don't agree, don't select Continue. You can't use this online check-in without agreeing. Contact your clinic, which can help you check in another way and handle any needed medical-record updates manually.
12. Changes to these Terms
If these Terms change, we will show you the new version and ask you again. Your earlier answer does not carry over to a new version.
13. Governing law
These Terms are offered by Visentia LLC and are governed by the laws of the State of New Jersey.
14. Questions
For questions about your appointment or this check-in, contact your clinic.
